REQUESTS FOR DETERMINATION OF APPLICABILITY

James Tietje, 352 Scranton Avenue, Falmouth, MA – For
 permission to remove a 2000 gal underground storage tank
 and fiberglass fuel line, check soils for contamination,
 back fill and pave over.

CONTINUED REQUESTS FOR DETERMINATION OF APPLICABILITY

Sandra Joseph, 42 & 44 Drift Road, West Falmouth, MA – 
For permission to fill vertical voids on the top of the 
stone riprap and chinking the entire face, all work by hand.

REQUESTS FOR HEARING UNDER A NOTICE OF INTENT

Town of Falmouth – Engineering Department (c/o Peter McConarty),
 Chappaquoit Road (Map 24-06-004-000; Map 24-06-003-000; Map
 24-07-008-000; Map 24-06-007-000B; Map 24-06-006-000; Map 24-06-005-000; Map 24-06-008-000), West Falmouth, MA – For permission to
 repair the existing seawall by installation of bedding 
stone and geotextile filter fabric, reconstruction of
 riprap stone revetment, installation of steel sheet
 pilings, steel guard rails, repaving and the associated 
clearing, excavating, grading and landscaping.

Augustine & Denise Centola, 123 Shorewood Drive, East Falmouth, MA –
 For permission to demolish the existing dwelling and
 construct a new single-family house with an attached
 deck, farmer’s porch, install dry wells, utilities, 
driveway, native mitigation plantings and the associated 
clearing, excavating, grading and landscaping.

CONTINUED REQUESTS TO AMEND THE EXISTING ORDER OF CONDITIONS

Phillip Cavallo, 297 Quaker Road, North Falmouth, MA – Request
 to amend the existing Order of Conditions (DEP 25-3764) 
to modify the existing deck, reconstruct steps, incorporate
 a landing and install a stone wall.

CONTINUED REQUESTS FOR HEARINGS UNDER A CERTIFICATE OF COMPLIANCE

Nantucket Sound Trust, Lots 3 & 4, Davis Neck Road, East 
Falmouth, MA – Request that a certificate of compliance be 
granted for the sea wall reconstruction under DEP 25-3682.

CONTINUED ENFORCEMENT ISSUES

Black Beach Harbor Head Association, William Reznikoff, Steven 
Doyle, Lawrence Lynch Corporation, the entire length of the 
dirt roadways for Little Neck Bars Road, Black Beach Hills 
Road and Drift Road, West Falmouth, MA – For unpermitted
 work within a resource area buffer as defined in FWR 10.00 
and 310 CMR 10.00.
