REQUESTS FOR DETERMINATION OF APPLICABILITY
Virginia Richardson Trust (c/o Lisa Palmer), 187 Teaticket Path, East Falmouth, MA – For permission to vista prune and to remove two dead black oaks.
Steven Garfinkle, 24 Racing Beach Avenue, Falmouth, MA – For permission to re-grade and repave a portion of the driveway, to install retaining walls, a new gas service and the associated clearing, excavating, grading and landscaping.
The 300 Committee (c/o Jack Sidar), Rydal Mount Drive (Map 39-23-001-000), Falmouth, MA – For permission to re-establish and re-route an existing trail, remove debris and invasive plants and to install an informational kiosk.
Town of Falmouth – DPW (c/o Gerry Potamis), 154 Blacksmith Shop Road, Falmouth, MA – For permission to re-grade the existing bituminous concrete access road north of existing beds No. 12 and 13 and overlay with pavement, to transition to gravel road, to create a new 12-ft wide access road including grubbing, clearing, grading and paving and the associated clearing, excavating, grading and landscaping.
REQUESTS FOR HEARINGS UNDER A NOTICE OF INTENT
Quayside Condo Owners, 93 Falmouth Heights Road, Falmouth, MA – For permission to replace, install and maintain a pressure-dosed sewage disposal, reconstruct existing sidewalks, reconfigure and pave the parking area, install gas service, keystone unilock block units, utilities and the associated clearing, excavating, grading and landscaping.
Philip Chischpotich, 189 Surf Drive, Falmouth, MA – For permission to raze the existing dwelling and construct a new house on open piers with a deck, stairs, to reconnect utilities, install gravel driveway and the associated clearing, excavating, grading and landscaping.
Estate of Robert Bryant, Pabian & Russell, LLC (c/o Jay Pabian), 54 Sharon Ann Lane, East Falmouth, MA – For permission to modify, maintain and license the existing timber pier, ramp and float, repair/install water and electrical utilities, after-the-fact permission for the exiting pergola & patio, restoration and mitigation plantings required to satisfy an outstanding Order of Conditions under DEP 25-1083 and the associated clearing, excavating, grading and landscaping.

CONTINUED REQUESTS FOR HEARINGS UNDER A NOTICE OF INTENT/REQUEST FOR VARIANCE
David and Martha Casiles et al, #0 Ferry Road (Map 23-20-010-179), Waquoit, MA – For permission to construct and maintain of timber stairs, a 4.5-ft x 82-ft fixed timber pier, 3-ft x 14-ft ramp, 4-ft x 10-ft and 5-ft x 12-ft float, four 4 x 4-inch posts, two 6 x 6-inch posts and thirteen 12-inch diameter pilings for shared community access and the associated excavating and grading.
Request for a variance under FWR 10.13 from the performance standards set out in FWR 10.16(1)(b)(6).
REQUESTS TO AMEND THE EXISTING ORDER OF CONDITIONS
John & Maria Maxwell, 13 Ocean Avenue, East Falmouth, MA – Request to amend the existing Order of Conditions (DEP 25-3871) to modify the patio, install stepping stones, mitigation plantings, remove non-native plant material and install native plants and the associated clearing, excavating, grading and landscaping.
