The Falmouth Conservation Commission will hold a public meeting & a public hearing on Wednesday, May 1, 2013 at 7:00 P.M. to discuss the following:

REQUESTS FOR CONTINUANCE UNDER HEARINGS FOR NOTICE OF INTENT

James Luccio, Lot 532, Acapesket Road, East Falmouth, MA – For permission to construct a new 4-bedroom single-family house with an attached garage, deck, deck with stairs, to install a retaining wall, Title 5 sewage disposal system, dry wells, driveway, trench drain, utilities and the associated clearing, excavating, grading and landscaping.

REQUESTS FOR DETERMINATION OF APPLICABILITY

Patricia Margolis, 15 Lake Shore Drive, East Falmouth, MA – For permission to remove the existing system, install a new Title 5 sewage disposal system and restore the driveway.

William Blumenreich, 61 Menauhant Road, East Falmouth, MA – For permission to remove 82-ft of cap on riprap retaining wall, drill rebar into existing wall, install forms wall to existing dimensions, pump 4000 lbs of concrete, fill back side of wall with 6-inch chinch stone no greater than 4-ft from the wall, install crushed blue stone, remove wind-blown sand from under house and against house, repair erosion at driveway and remove all debris created from scope of work.

John Dow, 27 Madeline Road, East Falmouth, MA – For permission to pump, fill and remove the existing cesspools and install a new Title 5 sewage disposal system.

REQUESTS FOR HEARING UNDER A NOTICE OF INTENT

James & Mary Catherine Mammary, 42 Brainerd Road, North Falmouth, MA – For permission to reconfigure the existing licensed ramp and float, the installation of four (4) float pilings, the installation of a 4-ft x 4-ft concrete landing for the landward end of the ramp and the associated excavating, grading and landscaping.

Gail Bambrick, 12 Circle Road, Waquoit, MA – For permission to remove the existing failing wood retaining wall and dumped stones, deck and patio and to replace and reconstruct in the same area but with a smaller deck, patio and retaining wall and the associated clearing, excavating, grading and landscaping.

Robert Rotondi, 92 Edgewater Drive East, East Falmouth, MA – For permission to renovate and expand the existing single-family house with two new additions, deck expansion, reconfigure the driveway and expand parking, to install a covered portico, sidewalk, retaining walls, AC units, utilities, dry wells, native mitigation plantings, to repair and expand the existing dock and the associated clearing, excavating, grading and landscaping.

CONTINUED REQUESTS FOR HEARING UNDER A NOTICE OF INTENT

Michael George, 45 Dusty Miller Road, Falmouth, MA – For permission to construct and maintain two house additions, a porch addition, to remove and abandon a portion of the existing driveway to be converted to lawn, construction of a new driveway, retaining walls and steps, installation of a new Title 5 sewage disposal system, drainage, utilities, mitigation and restoration plantings and the associated clearing, excavating, grading and landscaping.

Gail Bambrick, 12 Circle Road, Waquoit, MA – For permission to demolish and remove the existing wooden retaining wall, stones, cinder blocks, deck and patio and reconstruct a deck with footings, patio, vinyl sheeting retaining wall, install 12-inch diameter fiber rolls, native woody plants and the associated clearing, excavating, grading and landscaping.

Anastasios Parafestas, 65 Falmouth Heights Road, Falmouth, MA – For permission to dredge a 9000 sq ft area to a depth of 10-ft by removing approximately 300 cubic yards of material from Falmouth Inner Harbor for the benefit of 65 Falmouth Heights Road. The project includes the harvest and relocation of shellfish from the dredge area, dewatering the material on a barge, deposition into trucks and transportation to another waterfront location for beach nourishment and to create coastal dune.

Black Beach Harbor Head Association, Little Neck Bars Road, Black Beach Hills Road, Gilbert Lane and Drift Road, West Falmouth, MA - For permission to implement proposed road maintenance and storm water management improvements that includes vegetated swales and addition of dense grade crushed stone, along approximately 1 mile of private gravel roads and the associated clearing, excavating, grading and landscaping

REQUEST TO AMEND THE EXISTING ORDER OF CONDITIONS

Rose & Robert, 23 Green Pond Road, East Falmouth, MA – Request to amend the existing Order of Conditions (DEP 25-3863) to modify the house reconstruction with two additions, a new entry and stair tower and other modifications.
