The Falmouth Conservation Commission will hold a public meeting & a public hearing on Wednesday, August 21, 2013 at 7:00 P.M. to discuss the following:

REQUESTS FOR DETERMINATION OF APPLICABILITY

Dick K. P. & Eva W. Yue, 45 Nemasket Road, West Falmouth, MA - For permission to demolish and remove the existing house and cottage and to construct a single-family house with an attached garage, decks, ramp, porches, in-ground swimming pool, spa, reconfigure the driveway, install dry wells, a new Title 5 sewage disposal system, retaining wall, maintain the existing subsurface irrigation system and the associated clearing, excavating, grading and landscaping.

Stephen & Rosemary Fassett, 80A Little Island Road, West Falmouth, MA – For permission to install a 500 gal urine storage tank for the Eco Toilet Incentive Program.

Clive Gummow, 45 Sunset point Road, North Falmouth, MA – For permission to install a 25-ft wide native shrub buffer, a temporary above-ground irrigation system, to monitor the plantings for 3 full growing seasons, maintain a 4-ft path, to remove unpermitted gravel and replace with loam and seed, to comply with the expired Order of Conditions (DEP 25-2152) for house construction.

REQUESTS FOR HEARING UNDER A NOTICE OF INTENT

Peter Donovan, 7 Hamblin Point Road, Waquoit, MA – For permission to construct a 4-bedroom single-family house with an attached garage, deck with stairs, porch, stone wall with stairs, to install dry wells, utilities, a Title 5 sewage disposal system, driveway, lawn, native shrubs and the associated clearing, excavating, grading and landscaping.

Stephen Kaufman, 71 Childs River Road, Waquoit, MA – For permission to reconstruct and maintain the existing in-ground swimming pool, patio, pool shed, install a spa within existing pool patio footprint, dewatering basin, provide construction access and the associated clearing, excavating, grading and landscaping.

CONTINUED REQUESTS FOR HEARING UNDER A NOTICE OF INTENT

James Luccio, Lot 532, Acapesket Road, East Falmouth, MA – For permission to construct a new 4-bedroom single-family house with an attached garage, deck, deck with stairs, to install a retaining wall, Title 5 sewage disposal system, dry wells, driveway, trench drain, utilities and the associated clearing, excavating, grading and landscaping.

Clive Gummow, 45 Sunset Point Road, North Falmouth, MA – For permission to install a 25-ft wide native buffer strip, maintain a 4-ft path, install an above-ground irrigation system, submit monitoring for plant health, remove unpermitted gravel seaward and along the sides of the house, maintain a 4-ft gravel path and the associated clearing, excavating, grading and landscaping. Proposed work activities are to address an expired Order of Conditions (DEP 25-2152).

123 Highland Avenue Realty Trust (c/o Susan Knute, Trustee), 123 Highland Avenue (Lots 20, 21, 22, 23, 24, 25 & 27) and Highland Avenue west of Arlington Street, North Falmouth, MA – For permission to construct a duplex house with two dwelling units and maintaining one dwelling pursuant to M.G.L. Chapter 40B with garage, decks, to connect to Town sewer and to construct a new water service to abutting residential lots along Highland Avenue and the duplex house, to construct a gravel drive and parking area, gravel driveway on Glen Avenue and the associated clearing, excavating, grading and landscaping.

REQUESTS FOR HEARINGS UNDER AN ABBREVIATED NOTICE OF RESOURCE AREA DELINEATION

Carolyn Matzkin, 1487 Sandwich Road, Falmouth, MA - Request for confirmation of the following wetland resource area boundaries: BVW/freshwater wetland and isolated vegetated wetland.

REQUESTS TO AMENT EXISTING ORDER OF CONDITIONS

John & Laura Francis, 195 Great Bay Street, East Falmouth, MA – Request to amend the existing Order of Conditions (DEP 25-3929) to construct a breezeway connection between the house and the garage.

REQUESTS FOR HEARING UNDER A CERTIFICATE OF COMPLIANCE

Katherine Johnson, 16 Clay Path, Waquoit, MA – Request to allow the in-ground irrigation system installed during house reconstruction under DEP 25-3605.
